Question: Our Germand Shepherd suddenly became lethargic, with no appetite, but an unquenchable thirst, and began having HUGE urine accidents indoors (never happened before.) Eventually, he just layed down and did not want to get up. We took him to vet, and a quick urinalysis detected keytones in his urine. Could this be due to the recently tainted dog foods?

Unlikely. Keytones in the urine is a sign of diabetic conditions. So is the increased water consumption and the accidents. How old is the dog? If more than 8 years old then that could be a natural byproduct of age and a crappy diet. If younger than 8 years then there is a remote possibility. The reason that I say remote is that the general symptomology of poisoning is liver failure before pancreatic failure. Liver failure usually doesn't lead to keytones in the urine and Liver failure would have brought out other issues as well.

DKA results from an absolute shortage of insulin; in response the body switches to burning fatty acids and producing acidic ketone bodies that cause most of the symptoms and complications.[1] DKA may be the first symptom of previously undiagnosed diabetes, but it may also occur in known diabetics due to a variety of causes, such as intercurrent illness or poor compliance with insulin therapy. Vomiting, dehydration, deep gasping breathing, confusion and occasionally coma are typical symptoms. DKA is diagnosed with blood and urine tests; it is distinguished from other, rarer forms of ketoacidosis by the presence of high blood sugar levels. Treatment involves intravenous fluids to correct dehydration, insulin to suppress the production of ketone bodies, treatment for any underlying causes such as infections, and close observation to prevent and identify complications.[1][2]

When your body cannot absorb the sugar you eat and produce, you won't be able to have as much energy as you used to. Eventually, your body will stop turning to sugar for energy, and will result to fat instead. This is one of the reasons that a symptom of diabetes is weight loss. When too much fat is broken down, ketones develop. DKA stands for Diabetic KetoAcidosis. This is when your body has so many ketones that it begins to function abnormally. The ketones cause the DKA, not the other way around.

Sound very much like diabetes, or more specifically diabetic ketoacidosis. That would not have been caused by the food, but rather by his body's inability to produce insulin correctly.

Not being sick and eating normally would not make ketones present in your urine. I don't think weight loss would either. It's very common to lose weight in the first trimester. Ketones are present in the urine either when you have gestational diabetes or if you haven't eaten/drank enough- then your body starts to burn the fats in your body. If, as you say, you'd eaten and drank before the urine test then the ketones could indicate gestational diabetes. It may be that you've already had GD and not known with your previous pregnancy as your son was quite large at birth. I know this sounds horrible and I don't like to be the bearer of bad news, but even if you did have GD, it's very manageable with diet change and it goes away as soon as the baby is born. Managed properly it'll mean your baby will have a healthier start and there's no real risk to your baby if you do as the dietitician advises. Whilst waiting for your test, if you haven't already it might be advisable to cut down on sugary foods. Not give them up completely, but just cut down. I was diagnosed with GD (it turned out they mixed up my bloods and I don't) but I decided not to eat anything too sugary to be on the safe side. My dietitician also advised me not to look up gestational diabetes online as a lot of websites have scare tactics and lots of incorrect data. Anyway, best of luck with your pregnancy and congratulations. Again, I really hope my answer wasn't too upsetting but even in the worst case scenario, there's nothing much to worry about.
